Strategies to Improve Hypertension Control

Chelsea Redman (Associate Director, Health Equity Alliance) and Yely Montano (Senior Analyst, Health Equity Alliance) share insights into three innovative health system hypertension initiatives. Judy Hannan (Senior Advisor, CDC/CMS Million Hearts ®) and Hilary K. Wall (Senior Health Scientist/Science Lead, CDC/CMS Million Hearts ®) joined the Alliance to provide an overview of the initiative’s health equity priorities, as well as lessons learned from their partnership with federally qualified health centers to implement evidence-based hypertension protocols.
